Custom Ranks

Hello all. I have a question involving ranks. I couldn't help but notice some seemingly have custom ranks, even though they have fewer post then, say, I do. Is this something you ask for? Is it something you earn/pay for? Because if there was any rank I could have, well, if you know me at all, there would only be one tht made any kind of sense:
